Congenital glaucoma, also known as infantile glaucoma, is a rare but critical eye condition affecting approximately 1 in 10,000 live births. It is characterized by an abnormal increase in intraocular pressure (IOP) shortly after birth. This occurs due to the failure of development or abnormal development of the eye's natural drainage system (the trabecular meshwork).
Most cases of primary congenital glaucoma occur sporadically. However, in about 10% of cases where a hereditary pattern is evident, it is generally believed to follow an autosomal recessive inheritance pattern. How is Congenital Glaucoma Detected?

Epiphora (Excessive Tearing): Persistent watery eyes not related to crying.

Photophobia: Extreme sensitivity to light. Infants will often bury their heads in pillows or turn away from bright room lights.

Blepharospasm: Involuntary squinting, blinking, or squeezing of the eyelids.

Enlarged Cloudy Cornea: The cornea (clear front window of the eye) appears abnormally large and takes on a hazy, blue-gray, or cloudy appearance instead of being clear and bright.

Treatment of Congenital Glaucoma
Unlike adult glaucoma, which is initially managed with eye drops, the primary treatment for congenital glaucoma is surgical. While drops may be used temporarily to lower pressure before surgery, they cannot cure the underlying developmental defect.
The aim of pediatric glaucoma surgery is to lower intraocular pressure either by improving the outflow of fluid (aqueous humor) from the eye or by decreasing its production. Common surgical procedures include:
- Goniotomy: A micro-procedure where the surgeon cuts the abnormal trabecular meshwork tissue to clear the drainage path. The success rate is highly associated with the age of the child at diagnosis and the severity of the glaucoma.
- Trabeculectomy: Creating a tiny new drainage channel or valve on the side of the eye to allow excess fluid to bypass the blocked meshwork.
- Glaucoma Drainage Implants: Inserting a microscopic tube to shunt excess fluid away and relieve eye pressure.
